In this agreement "You" and "Your" "Participant" means you the applicant, and "Us" and "We" means CardioCentrix.com.

Referral Percentages: Whenever a paying customer, referred by you, receives a screening service from us, you will receive 25% of their screening price each and every time they have a screen. (Exception: If you refer someone to a public screening event where the screening price is already discounted, you will receive 10% of the discounted screening price.) After you have referred 50 people who have paid for a DPA screening, you will receive 30% on your referrals thereafter. (Exception: If you refer someone to a public screening event where the screening price is already discounted, you will receive 10% of the discounted screening price.) In addition: You may earn 10% of product sales residually as long as customer continues to purchase product. The product referred to is CARDIO PURE cardiovascular nutrition. Cardio Party: A cardio party is a group of 4 or more people that you have gathered together in a single setting for the purpose of receiving DPA Screenings (at the appropriate discount. See price list.). For participants wishing to set up a Cardio Party the following applies:
1. If you have 4 or more paying guests, you will receive a 5^th screen free to be
redeemed at that event.
2. You may earn 10% of product sales residually as long as customer continues to purchase product. The product referred to is CARDIO PURE cardiovascular nutrition. For more information on CARDIO PURE please click here. (Also see "Payments" and Product Commission Guidelines below.) Also applies to other qualified products.

Recruiting Others: You may recruit others to assist you in making referrals. Any and all commissions paid to a recruit will be your sole responsibility. Cardio Centrix will not pay commissions to anyone other than those who sign up under the REP Program.

Validity of Referred Customers: You are entitled to commissions from ALL customers referred to us by you, even if they do not receive services for up to 1 year. Status: You can always check your referral status, or other information, by calling us at 1-480-767-3227.

Payments: Commission payments are mailed on the 15th of every month, or the following business day if falling on a weekend or public holiday. Minimum Payment: The minimum payment is $25.00. A lesser balance will be carried over to the following month unless you request otherwise. If a request is made to pay out a smaller balance, a $4 administrative fee will be deducted from commission payment. Charge backs/refunds: If for any reason we have to give a customer refund, this will reflect commission payment balance by way of a debit for that specific transaction and you will be notified by email and will be deducted from future commissions.

General: You are free to promote or advertise Cardio Centrix, Inc, however, spam is strictly prohibited. In addition, any use of logo or artwork must have the written permission of Cardio Centrix, Inc. No claims or information other than that available on the official CardioCentrix.com website may be used and is strictly prohibited. Under NO Circumstance is it permitted for you to express or imply that you are an employee or that you are certified/credentialed to perform screening tests as it would be a direct misrepresentation of Cardio Centrix, Inc. Cardio Centrix, Inc. reserves the right to amend this agreement at any time without notice. 

Title: REP stands for Referral Earning Program. "You" are a referral earning program participant. This does not mean that you are a "representative of Cardio Centrix Inc or that you may represent yourself as a "representative" of Cardio Centrix Inc as you are not employed by Cardio Centrix, Inc. A Cardio Centrix Inc representative will have a seperate employment agreement/contract and receive formal training, which is not a part of the REP Program.  Misrepresentation of your participation will result in immediate termination of your participation and forfeiture of residual earnings. You will be notified by email and certified mail of this status change.

Cancellation: You have the right to cancel your participation in the REP Program for any reason and at any time by emailing us at . Cancellation will result in forfeiture of residual income. As well, Cardio Centrix, Inc. may cancel this agreement at any time for breach of any portion of this agreement.  EMAIL

Remaining Balance: If you choose to cancel your participation, your remaining commission balance will be paid out at the normal time and if less than $25 the $4 administration fee will apply.

Term: This agreement shall remain in force until cancelled by participant or Cardio Centrix, Inc.  Terms and conditions are reviewed semi-annually.

Tax: All REP participants will be issued a W9 form that must be received by Cardio Centrix, Inc. prior to disbursements of commissions. REP participants are responsible for all income reporting to local and federal agencies. Deductions will not be removed from commission checks.
